To Apply

An application for burial assistance must be made no later than 10 business days after the burial, cremation or donation takes place.

Click on the MI Bridges button and fill in the necessary information.

Visit the local Department of Health & Human Services Office

Where do I get a funeral statement to submit?

Once you have applied, let us know and we will provide the Informant responsible for paying the funeral statement with any needed paperwork, which will include our provider identification.

Response

In our experience, families usually receive a response within 7-10 business days regarding their eligibility.  If approved, our funeral home will also receive an email notifying us of the amount approved.
